Our benefits package is in the top 1% in the state of Washington and includes Excellent medical coverage, dental, vision Paid time off (PTO) Retirement Professional development opportunities 0.6 to 0.8 FTE - Non-CBA position GENERAL POSITION SUMMARY : Develops and implements the hospice spiritual and bereavement support program that includes emotional, psychosocial, and bereavement counseling services provided before and after the death of the hospice patient to assist with issues related to grief, loss, and adjustment. Serves as a liaison between community clergy and Hospice. Plans, develops, coordinates and evaluates the hospice bereavement support program, including but not limited to individual counseling, support groups, memorial services, mailings, program announcements, program or event press releases, and community outreach activities. Provides short-term bereavement counseling to individuals referred from Jefferson Healthcare providers and community agencies within East Jefferson County Provides information and makes referrals to appropriate agencies for individuals needing crisis and long-term counseling services as appropriate Participates in the hospice interdisciplinary team/group (IDT/IDG) conferences to assure that spiritual, loss and grief needs are continually monitored and addressed. Participates in the bereavement assessment process, care planning and goal clarification and assists in the identification and support of individuals at risk of complicated or acute grief Provides education and support to the hospice interdisciplinary team , the hospice volunteers and other staff regarding the grief process and bereavement related issues Develops and maintains community grief resource materials for use by hospice staff, hospice patients/ families and support group attendees Documents spiritual and bereavement care provided to hospice patients/families including support Education and experience desired: Bachelor's degree in theology , social work, or divinity required . Master's Degree in a related area, such as divinity, social work, theology preferred. Two years of experience working in a hospice program as a bereavement/grief counselor, chaplain, medical social worker, or nurse . Experience leading/ facilitating support groups. Bereavement support group facilitation experience strongly preferred. Certification in bereavement counseling is required if Bachelor's or Master's Degree in an unrelated field . Certification in bereavement counseling to be obtained within one year of hire Current valid Washington State automobile driver's license and proof of automobile insurance. Job Posted by ApplicantPro
